As a client/participant of Flourishing Minds Wellness Center, you have the

incomplete information, you may request corrections.
>> Confidential Communications: You may request that we contact you in a
specific manner (e.g., home or work phone, specific mailing address).
>> Restriction Requests: You may request limits on the use or sharing of your PHI;
however, we may not always be able to grant these requests if they impact your
care.
>> Accounting of Disclosures: You may request a list of times we have shared
your PHI, except for treatment, payment, and healthcare operations.
>> File a Complaint: If you believe your privacy rights have been violated, you have
the right to file a complaint without retaliation.
